Important notification about APC plan for 2025

Ascension Personalized Care will no longer be offering plans on- or off-exchange in Indiana, Kansas, Tennessee and Texas beginning January 2025. Members will continue to have coverage with Ascension Personalized Care through December 31, 2024. 

 

Individuals currently covered by Ascension Personalized Care will automatically be mapped to a similar insurance plan during the Open Enrollment Period for 2025 and should not experience a gap in insurance coverage. Covered individuals may also choose to self-select a different insurance plan during that same timeframe. 

 

It is a priority for Ascension Personalized Care to continue to provide compassionate, personalized care and to deliver a positive experience to the members, doctors, brokers, and other impacted parties. We want to ensure all members are able to move to new plans with no interruption in coverage and care.

Is Ascension Personalized Care going away in every county or state?

Ascension Personalized Care will no longer be offered in any county or state in 2025.

 

When will coverage end?

Coverage with Ascension Personalized Care will end on December 31, 2024.

 

Can you make an exception for some?

No. We cannot make any exceptions for any members.

 

When can members enroll in new Affordable Care Act Individual and Family coverage for 2025?

The Health Insurance Marketplace Open Enrollment Period for 2025 is from November 1, 2024, through December 15, 2024, for a January 1, 2025, effective date. During the Open Enrollment Period members have the opportunity to visit healthcare.gov to review, compare and select a new policy for 2025.
